NEW STUDENT INFORMATION FORM

The New Student Information Form is required before MIS can consider your student for admission.

Please print and deliver the New Student Information form to your current school office for them to fax or mail back.  If applying to 2nd through 8th grade, the student’s current school will also need to send report cards, attendance, and conduct records.
